Multicentre, prospective, UK, observational cohort study with propensity-matched analysis to determine optimal allocation of surgery plus ET (S+ET) or PET in women aged ?70 with breast cancer. Data on fitness, frailty, cancer stage, grade, biotype, treatment and quality of life were collected. Propensity-matching (based on age, health status and cancer stage) adjusted for allocation bias when comparing S+ET with PET.
A total of 3416 women (median age 77, range 69-102) were recruited from 56 breast units-2854 (88%) had ER+breast cancer 2354 had S+ET and 500 PET. Median follow-up was 52 months. Patients treated with PET were older and frailer than patients treated with S+ET. Unmatched overall survival was inferior in the PET group pectancy (&gt;90 years or &gt;85 with comorbidities or frailty) individualised decision making regarding PET versus S+ET may be appropriate and safe to offer. ISRCTN 46099296.The present study examined the cognitive locus of Stroop dilution using a psychological refractory period (PRP) paradigm. Participants were asked to perform a tone discrimination task via a bimanual keypress response and a modified Stroop task via a vocal response serially as Task 1 and Task 2, respectively. In Task 2, a neutral word was added on half of the trials and no neutral word on the other half of the trials to observe the Stroop dilution effect. The amount of Stroop dilution, as well as the Stroop effect, was relatively constant across different stimulus-onset asynchronies (SOAs), which implies that Stroop dilution occurs due to the competition between a neutral word and a color word after a target color bar is selected to be processed further. These results indicate that focused attention plays an important role in the modulation of Stroop interference by the presence of a neutral word.Within autobiographical knowledge, semantic and episodic memory are traditionally considered separate, but newer models place them along a continuum, which raises the possibility of an intermediate form of knowledge - personal semantics. This study tested how different types of semantics - general semantics and two forms of personal semantics - impact access to personal episodic memories. In two experiments, participants made a series of true/false judgments about a prime statement, which reflected a general semantic fact, a context-dependent (e.g., repeated event) or context-independent (e.g., trait), personal semantic fact and then retrieved a specific past episodic memory. There was a significantly stronger priming effect for accessing specific episodic memories after judging personal semantic facts versus general facts. We also found that context-dependent and -independent personal semantic facts had separable priming effects on episodic memory. This study investigates the feasibility of using dedicated MR-compatible ionization chambers at MR-integrated radiotherapy devices. MR-compatible ionization chambers (Exradin A19MR, A1SLMR, A26MR, A28MR) were precisely modeled and their relative response in a 6MV treatment beam in the presence of a magnetic field was simulated using EGSnrc. Monte Carlo simulations were carried out with the magnetic field in three orientations the magnetic field aligned perpendicular to the chamber and beam axis (transverse orientation), the magnetic field parallel to the chamber as well as parallel to the beam axis. Monte Carlo simulation results were validated with measurements using an electromagnet with magnetic field strength upto 1.1 T with the chambers in transverse orientation. The measurements and simulation results were in good agreement, except for the A26MR ionization chamber in transverse orientation. The maximum increase in response of the ionization chambers observed was 8.6% for the transverse orientation.
